ABOUT 4D TRANSPORT

4D Transport provides non-emergent wheelchair and ambulatory transportation services to individuals.  We pick-up from private homes or medical facilities in Louisiana. 

Our service area includes Tangipahoa Parish, St. Tammany Parish, Livingston Parish, and other surrounding areas.

 

We provide transportation to and from medical appointments/procedures, kidney dialysis, hospital discharges to home or skilled nursing facilities. 

 

We currently work with the following medical facilities: Landmark Nursing Center Hammond, Heritage Manor Of Mandeville, Forest Manor of Covington, Trinity Trace Community Care Center, Christwood Covington,  St. Tammany Parish Hospital, Lakeview Hospital Covington, Slidell Memorial Hospital, Our Lady Of Angels Hospital Bogalusa, North Oaks Hospital Hammond.

Note: Contracts with facilities are available .

 

We also provide wheelchair transportation to special events such as weddings, funerals, family gatherings, "home for the holidays."

 

Normal business hours are Monday through Friday, 7:00am-5:00pm.  We are available after-hours and on weekends by appointment.
